Medicare-supported osteopathy under a GP Care Plan
If you have an ongoing health condition, your GP may refer you for osteopathic treatment under a GP Chronic Condition Management Plan, commonly called a GP Care Plan, CDM plan or EPC plan.
At Peter Petroff Osteopathy in Yarraville, I accept eligible Medicare referrals and aim to make the process straightforward. Your appointment may include a detailed assessment, hands-on treatment, practical advice and an exercise program tailored to your condition and goals.

What is a GP Chronic Condition Management Plan?
A GP Chronic Condition Management Plan helps coordinate care for people with chronic or ongoing health conditions.

Your GP determines whether you are eligible and whether osteopathy is appropriate for your needs. Under the plan, eligible patients may receive Medicare benefits for up to five individual allied-health services per calendar year. These visits are shared across all eligible allied-health providers rather than automatically providing five osteopathy appointments.

For example, your GP may allocate:

five osteopathy consultations; or
-
three osteopathy consultations and two podiatry consultations.
Can osteopathy be included?
Yes. Osteopaths are among the allied-health professionals who may provide services under a GP Chronic Condition Management Plan.
The treatment must relate to the chronic condition being managed under your plan.

Your GP decides whether an osteopathy referral is suitable for you.
How the process works
1. Speak with your GP
Ask your GP whether you may be eligible for a GP Chronic Condition Management Plan and whether osteopathy would be appropriate.

2. Obtain your referral
Your GP must provide a valid referral for osteopathic services.

3. Book your appointment
Book online or contact the clinic and mention that you have a Medicare GP Care Plan referral.

4. Bring your documents
Please bring:

your GP referral

your Medicare card
-
relevant scans or medical reports
-
a list of current medications, where relevant

5. Claim your Medicare benefit
At the end of your consultation, you will be required to pay the consultation fee of $100. Your Medicare rebate of approx. $61.80 will automatically be refunded to your nominated bank account in 1-2 business days.​
